Abstract: 

Introduction & Objective: There is rising variety of fillers used in aesthetic surgery for skin augmentation and enhancement. The natural fillers include free Fat Graft, derom-Fat Graft and reverse dermo-Fat Graft (dermis down and Fat up). There is not a unified opinion on comparative longevity for these Grafts among authors. The purpose of this article is to find out the out the come of these Grafts in the animal model.Materials & Methods: The study was performed at St. Fatima hospital animal laboratory on 12 Spraugue Dawly male rats who weighed 300 to 350 grams. Three forms of auto Fat Graft including free Fat, dermo-Fat and reverse dermo-Fat was applied and after two months animals were sacrificed. The specimens were weighed before and after Grafting with precise digital scale. All specimens stained with H& E dye and studied for the amount of fibroblast proliferation, collagen bundles, necrosis, neovascularization and eosinophil and giant cell infiltration.Results: We found statistically no significant difference of the pre and post Graft weight changes between these three groups. We also couldn’t find and significant differences between the amount of fibroblast proliferation, collagen bundles, necrosis, neovascularization and eosinophil and giant cell infiltration.Conclusions: These findings show that free Fat can be used as filler with no need to be combined with dermis. On the other hand we found out that derom-Fat take and outcome is not dependent on the position of dermis whether to be over or under the Fat Graft.

Abstract: 

Osteogenesis, Osteoiduction, Osteoconduction are three essential elements of bone regeneration along with final biding between host bones and Grafting material.In this study, six-month-old male New Zealand rabbits with a body mass of about 2.5 kg were used. 15 rabbits randomly divided into three groups. In group I in both hands ulna bone was defected (3*1.5 mm) without any treatment. Just some omental and white Fat cutted and removed. In group II ulna bone of right hand was defected (3*1.5 mm) and omentum of animal itself was used for Grafting. In group III ulna bone of left hand was defected and white Fat abdominal was used for Grafting. Movements of animal were evaluated in days 3, 6, 9, 12, 15, 20, 25, 30, 40 of observation.In all three groups First radiography was done just after operation. And also in days 15, 30, 45 of experiment. According to data obtained all three type of callus in group II were vastly generated. Osteogenesis procedure in omentum group was higher than other groups.Histopathology results in group I showed Perriosteal and modularly bridging callus beside bone marrow and fibroses tissues were vastly generated. In group II Osteogenesis in sponge form and blood vascularatin was so much and small amount of Fibroses tissues were seen. Furthermore Perriosteal and modularly bridging callus were noticed. In group III Between fibroses tissue some Fat tissue were also visible. Along with fibroses tissue Osteogenesis was in progress. Fibroblasts changed to octeocytes. Result obtained from histomorphometry indicates that differences among groups were meaningful (P-value <.05) and Osteogenesis procedure in group II was higher than other groups 72.3±2.02.It seems, in future to speed up osteo repairs omental Graft will be more extensively used. Because this method gives all desired advantages. Furthermore based on the results, to avoid adhesion Fat Grafting will be rejected.

Abstract: 

Background and Aim: The oroantral fistula is one of the complications that might occur following extraction of upper posterior teeth. The use of pedicled buccal Fat pad Graft has recently been considered in the treatment of oroantral fistula. The aim of this article was to evaluate using pedicled buccal Fat pad flap for closure of oroantral communications.Methods & Materials: In this experimental study (case series), thirteen patients (11 male & 2 female) with oroantral communications, which either developed following the extraction of maxillary posterior teeth or odontogenic cyst removal, with an average age of 34.06 (24-52 years old), were treated with a pedicled buccal Fat pad Graft. Surgeries were performed under local or generalized anesthesia. All of patients were evaluated for maxillary sinus infection before and 6 months after surgery with Water's radiography. Clinical repair of wound was evaluated at the first and the second week and 6 months after the operation.Results: Oroantral communication was treated successfully in all patients. Only one patient complained trom pain and nasal obstructions six months following surgery.Conclusion: Pedicled buccal Fat pad Graft can be successfully used for closure of oroantral communication.

Abstract: 

Introduction: The elbow is frequently involved in trauma and is one of the most frequently radiographed joint in emergency ward. Posterior Fat pad displacement has been describing in a variety of disorder such as hemophilia, rheumatoid arthritis and any condition that concomitant with joint effusion such as trauma. So it may be the manifestation of an Occult fracture as a result of trauma. Objective: The purpose of this study was to assessment the implication of posterior Fat-pad sign in occult elbow fracture. Materials and Methods: This research is a prospective and descriptive study that carried out on 328 patients with the age under 18 years old and a history of traumatic elbow injury. All of patients with the radiographically positive posterior Fat pad sign, without any intraarticular fracture were introduced in study. After 3 weeks the injury, anteroposterior, lateral, and oblique radiographs were made and evaluated for evidence of fracture healing (Periostial reaction). If there was evidence of new bone formation on any of this three radiographs, it was considered to indicate a fracture of elbow. Result: From 328 patients, 73% (242) were boys and 26% (86) were girls. 29(96)% of patients were posterior Fat pad positive sign that 85% (82) of above patient after 3 weeks become positive they had evidence of a fracture and in 18% (14) of patient hadn’t any evidence of fracture. 82% (65) of patients had supracondylar fracture, 41% (5) a fracture of lateral condyl humerus, 1.64% (2) a fracture of medial condyl humerus.Conclusion: This study demonstrated that the prevalence of posterior Fat-pad sign with the evidence of fracture in elbow is very high and need to evaluate the patients with occult fracture correctly is very important.

Abstract: 

Background: This study aimed to assess dermis Fat Graft (DFG) as a choice to correct the tissue deficit in the free border of the upper lip in cleft lip repair surgery. Materials and Methods: Thirty-five individuals who referred to Alzahra Hospital at 2013–2014, with lip deformity following the primary repair surgery of cleft lip underwent surgery by DFG technique. Outcomes were assessed 4 months after the surgery based on comparison of preoperative and postoperative photographs.Results: the results in 18 (51.42%) patients were excellent, 10 (28.57%) good, and 7 (20%) intermediate according to the satisfaction of patients and investigators in terms of filling of lip deficit and motion of the upper lip. Moreover, complications and pain were minimal after 4-month follow-up.Conclusion: This method introduces an admissible method with 80% good to excellent results based on satisfactory of patients and surgeon.

Abstract: 

Background: Autologous Fat Grafting in pediatric patients is limited due to restricted Fat tissue reserves. Preadipocytes, derived from adipose tissue, offer a promising alternative for soft tissue augmentation in children. Objectives: This study aims to compare the effectiveness of preadipocyte Fat Grafting with conventional adipose Grafting, particularly in cases where adipose tissue availability is limited. Methods: In this experimental study, 28 male Wistar albino rats were used. Fat was harvested from periepididymal Fat pads to prepare preadipocyte and adipocyte suspensions. These suspensions were injected subcutaneously into the right and left scapular areas, respectively. The rats were divided into four groups based on post-injection excision timing. Volumes of excised tissue were calculated using a hemi-ellipse formula. Histopathological examinations were conducted, comparing necrosis, fibrosis, vascularity changes, cyst formation, and inflammatory cell density. Results: Statistical analysis was performed using GraphPad Instat 3. 0 and SPSS 17. 0 software. Histopathologically, preadipocyte Grafts showed better scores for necrosis, fibrosis, cyst formation, vascularity, and inflammation across all groups. Although no significant volumetric differences were observed in the other groups, in the fourth group (22 weeks), volumetric loss of adipocyte Grafts was significantly greater than that of the preadipocyte Grafts (0. 09 ± 0. 01 vs. 0. 05 ± 0. 02, P < 0. 05). Conclusions: The findings suggest that preadipocyte Grafting holds promising potential for soft tissue augmentation, particularly in pediatric cases where limited Fat tissue availability presents challenges. Preadipocytes' inherent characteristics, including resistance to hypoxia and enhanced promotion of angiogenesis, may contribute to their sustained volume retention and improved histopathological outcomes. The significant reduction in adipocyte Graft volumes may be attributed to their susceptibility to hypoxia and Graft resorption. The ability of preadipocytes to withstand ischemic conditions and promote angiogenesis could contribute to their enhanced viability. Further research and clinical investigations are warranted to establish preadipocyte Grafting as a viable solution for soft tissue augmentation, especially in pediatric patients with limited adipose tissue availability. A comprehensive assessment of volumetric changes and histopathological outcomes provides deeper insight into the potential benefits of preadipocyte Grafting in pediatric healthcare.
